Momenta is dedicated to improving health outcomes through high quality, evidence-based programmes that support individuals in making sustainable lifestyle changes. We are seeking an enthusiastic and relationship focused Primary Care Engagement Lead to help advance this mission.
About the role
In this role, you will promote and facilitate patient referrals into our programmes by working closely with healthcare professionals, particularly within primary care. You will ensure clinical teams have a clear understanding of our services and feel confident in identifying and referring eligible patients. Your contribution will be central to increasing both the volume and quality of referrals across designated Integrated Care Board (ICB) areas. You will also play an important part in promoting equitable access, with particular attention to underrepresented patient groups.

What you'll be doing
Developing and maintain strong and supportive relationships with healthcare professionals in primary care to raise programme awareness and encourage appropriate referrals.
Providing clear and helpful guidance to clinicians and practice teams on the benefits of our programmes, patient eligibility, and referral expectations.
Working closely with the Key Account Manager to design and deliver strategies that support referral targets and key performance indicators.
Monitoring referral activity and quality across ICB areas, using data to identify opportunities for improvement and to support collaborative decision making.
Identifying GP practices where referrals from underserved or priority groups are low and implement engagement approaches that support more equitable access.
Leading training sessions, workshops, and presentations that build confidence and understanding among healthcare professionals, promoting high quality referral practices.
Delivering patient information events, both in person and through virtual platforms, to raise awareness of the service and encourage appropriate patient referrals.
Working with ICBs to present referral and outcome data along with other requested information, and use these insights to guide further engagement activity and targeted support.
Offering responsive ongoing support to healthcare professionals, addressing questions and challenges throughout the referral process.
Collaborating with the Key Account Manager and wider colleagues to develop and refine promotional materials that strengthen programme visibility and engagement.
Maintaining accurate records of engagement activity, referral data, and outcomes, ensuring timely reporting to the Key Account Manager.
Supporting the timely processing of referral information while maintaining high standards of data accuracy and follow up promptly on any missing or inaccurate information to ensure a smooth onboarding process.
You'll have
Experience in a relevant field such as public health is advantageous.
Strong interpersonal and communication skills, with the ability to build trust and influence healthcare professionals.
A target driven approach with the ability to meet performance expectations and achieve strong referral outcomes.
Understanding of primary care settings and wider healthcare systems, including referral processes.
Confident analytical skills with the ability to interpret data, recognise patterns, and recommend improvements.
Ability to work independently while also contributing effectively to team priorities and shared goals.
Excellent organisational skills, strong attention to detail, and the ability to manage multiple priorities.
Confident use of Microsoft Office and experience with customer relationship management systems.
Hopefully you'll also have
Knowledge of NHS structures and pathways.
Experience working within public health, lifestyle, or behaviour change programmes.
Experience delivering presentations to professional audiences.
Familiarity with customer relationship management systems and digital health platforms.
Ability to design and deliver training or workshops for healthcare professionals or external partners.
Understanding of the wider health and wellbeing landscape, including social prescribing and integrated care.
